The participation release form must be completed prior to attending Camp Tanager. This form verifies that you understand the program offered and give your consent for your child to participate.
This application is for children ages 7-13 with Type 1 or 2 Diabetes. Diabetes Camp offers a variety of recreational activities and opportunities for campers to acquire a greater understanding of their Diabetes management.

Camp Tanager participates in the USDA summer food service program. This form must be completed by all campers regardless of qualification of program status.
The health history form must be completed in full prior to attending Camp Tanager. This form provides vital information such as allergies, medications, and vaccination information.
The parent information sheet contains information such as what to bring, driving directions and contact information. This form can be printed and hung on the fridge for quick and easy access before and during your child's assigned camp week. Note: This form is designed for overnight camp only.
This application should be used for regular camp sessions held between June and August. Campers should be between the ages of 6-11 years. Note: The application must be handed into your child's school for processing. Do not return this form directly to Camp Tanager.
