Application for Overnight Camp

This application should be used for regular camp sessions held between June and August. Campers should be between the ages of 6-11 years. Note: The application must be handed into your child's school for processing. Do not return this form directly to Camp Tanager.

Parent Information Sheet

The parent information sheet contains information such as what to bring, driving directions and contact information. This form can be printed and hung on the fridge for quick and easy access before and during your child's assigned camp week. Note: This form is designed for overnight camp only.

Health History Form

The health history form must be completed in full prior to attending Camp Tanager. This form provides vital information such as allergies, medications, and vaccination information

Application for Diabetes Camp

This application is for children ages 7-13 with Type 1 or 2 Diabetes. Diabetes Camp offers a variety of recreational activities and opportunities for campers to acquire a greater understanding of their Diabetes management.

SmugMug Photo Form

View your child's special camp moments on our secure password protected photo site. You will need this form to sign up for a password to be able to view photos from the camp week.

Participation Release Form

The participation release form must be completed prior to attending Camp Tanager. This form verifies that you understand the program offered and give your consent for your child to participate.

Preparing your child for camp

Studies show that most kids experience some type of homesickness when away from home. This ACA information gives good advice for preparing your child for camp.
